This happens when you use Whiplash lol.

It can hit multiple targets in it's 5 yd radius and THIS IS NOT A HACK. This is on the Cataclysm Beta and it took blizzard about 3 days to fix it.

Whiplash is a spell used by Succubus and it has a 25 second CD.
